I read a letter in the 254th issue of dragon magazine about how something the size of a giant hitting a pc should result in more than just a loss of hp, with this in mind I designed the following rules on Knockback.
If a creature hits another creature at least 2 size categories smaller than it (even if they only hit the touch AC of said creature) with an upward or foward motion (such as a kick or a golf type swing of a club, or a dragons tail sweep) the creature that is hit is knocked away a number of feet equal to 5' or creatures height (whichever is lower) * str check result (explained later) and suffers falling damage for 1/4 of the total distance traveled.
If target creature hits a wall or other impending object (such as a wall of force) before it has traveled the total distance, it takes 1d6 damage per 10 feet traveled if it hits the obect before the apex of its "flight" or 1d3 points (you could just roll the same number of d6s and divide by 2 and round down) per 10 feet traveled if it hits after the apex (please note if it hits at the apex the creature takes damage as though it hit before the apex).
The apex of the creatures "flight" is reached halfway through the total distance traveled and is at a height of 1/4 the total distance traveled. (ex. a creature is hit 20 feet it reaches the apex at 10ft and the apex is 5 ft off the ground *ground being the elevation it was at when it was hit*)
the str check that determines the distance the creature travels is rolled when the larger creature hits, roll 1d20 and add their modified str bonus, the str bonus is modified according to these rules, if the hit creature is 3 size categories smaller than the attacker they get their str bonus to the str check at 4 size categories smaller they get 1.5 times their str bonus added to the str check and at 5 smaller 2 times, 6 smaller 2.5 times and so on.
ex. say a Storm giant, useing the stats from the MM, kicks a human, height 5'6", and hits. now to see how far the human is hit we roll a d20, say we get a 13, then since the human is 3 size categories smaller than the giant we add 1*Giants str bonus (his str is 39 so his bonus is 14) times 1 equals 14 so we have 13+14=27, then we multiply that by 5' since that is lower than the humans height, 27*5=135.
So the human is kicked 135 feet, halfway through the "flight" he reaches the appex (1/4 of total distance) 33.75 feet high at 67.5 feet out and falls to the ground at 135 away from the giant taking 30 feet worth of falling damage.
